Junior Club Payment Report (to transmit any manual memberships or manual payments)
Certificate of Insurance Request Form For practice facilities or tournaments. Complete & send to Region office--be sure to include facility address. Allow a couple of days for an e-mail reply with a printable certificate.
Background Screening Policy Falsification of information on any membership application or the consent/release form is grounds for membership revocation or restriction of membership. Individuals that are automatically disqualified must wait one season before reapplying for affiliation and/or participation with a junior club or team.
Incident Report Form: Your coaches should have blanks available at practices & tournaments in the event of a player/spectator/coach injury.
Spectator-Parent Code of Conduct If your parents & spectators aren't behaving well at your practices & tournaments, maybe it's because you haven't told them what is acceptable & unacceptable behavior. You may want to post this notice at your practice facility to help coaches understand what is considered acceptable interaction between coaches & parents.
Practice or Tryout Sanction Request Form Everybody still needs to be registered.

Universal Tournament Entry Form May be used to enter tournaments if you do not receive an entry form from the tournament host.

Background Consent Form: For any adult seeking a background check manually--unnecessary for on-line registration.
Non-Citizen Background Consent Form: For non-US Citizens who will be coaching or officiating (allow additional time)
Manual Membership Registration Form: Adults, players or officials--add $10 for manual registration
Junior Club Personnel Code of Ethics (only for manual coach registrations--sign & send to Region office)
Player Medical Release Form without Notary.

Chaperone Responsibility Form
Volunteer Waiver Release & Consent Forms to be used by clubs The Volunteer consent and release forms should be used for volunteers (all those who are not current members of the Delta Region) who assist with sanctioned events such as tournaments. To be used for guest coaches, clinicians, officials and all others serving in a volunteer capacity. Not intended for club coaches or other club staff. This form would serve as a first line of defense in case of an injury to the volunteer or possible lawsuit filed by the volunteer or on his behalf. Note also that the medical authorization would provide assistance in case the volunteer is incapacitated and unable to authorize medical treatment.
TOURNAMENT DIRECTOR FORMS:
Tournament Sanction Request Form This form is required to be submitted before tournament dates will be guaranteed. Form may be filled in using Acrobat Reader (use select text option) saved on your computer and submitted as an e-mail attachment to reg@deltavolleyball.net Tournament sanction fees (explained on the form) must be submitted before official sanction of event.
Report of Event To be submitted by Tournament Director after event
Spectator & Parent Code of Conduct Recommended to be posted at all events to govern behavior. Club directors may also want to post at team practices.
