2006-07 Memberships are valid only until October 31, 2007. If you have NOT renewed your membership, you are not covered by insurance after that date.
HOW TO REGISTER A JUNIOR GIRLS' VOLLEYBALL TEAM IN THE DELTA REGION:
Complete On-line Registration of Club Director & Coaches.
Get existing players to go on-line & renew their memberships, then either use the electronic signature option or print the waiver & release of liability. All players & parents to complete Medical Release form.
All Players & Coaches to complete additional forms as noted under "Online Registration"
Place registered players on individual teams and list on team roster form.
: Should be completed on any adult seeking a background check manually.
Some misdemeanors are now included in the background screening process. Notice that speeding tickets are not included. Single-offense DWI's would not be an automatic disqualifier. See actual language below.
Amend background screening policy effective November 1, 2005 to change Automatic Disqualifiers for participation in sanctioned Junior events and/or activities to: Convictions based on being found guilty, pled guilty; or pled nolo contendere for All Sex offenses regardless of time limit; Felony Violence and Felony Drug offenses in the past 10 years; any misdemeanor violence offenses in the past 7 years; any multiple misdemeanor drug and alcohol offenses within the past 7 years; or any other crimes against children.

Tournament Sanction Request Form This form is required to be submitted before tournament dates will be guaranteed. Form may be filled in using Acrobat Reader (use select text option) saved on your computer and submitted as an e-mail attachment to reg@deltavolleyball.net Tournament sanction fees (explained on the form) must be submitted before official sanction of event.
Report of Eventis to be submitted by Tournament Director immediately following completion of tournaments to report order of finish in event. Failure to send report promptly may result in loss of sanctioning for future events.
Volunteer Waiver Release & Consent Forms to be used by clubs - Please Read The Volunteer consent and release forms should be used for volunteers (all those who are not current members of the Delta Region) who assist with sanctioned events such as tournaments. To be used for guest coaches, clinicians, officials and all others serving in a volunteer capacity. Not intended for club coaches or other club staff. This form would serve as a first line of defense in case of an injury to the volunteer or possible lawsuit filed by the volunteer or on his behalf. Note also that the medical authorization would provide assistance in case the volunteer is incapacitated and unable to authorize medical treatment.
Example of forms & fees being gathered & sent to Region office by Junior Club Directors/Coaches:
1) Club Director gets all prospective coaches (including self) to complete registration forms on-line (including background screening) then submits to Region office with $35 per registered adult. Assuming no disqualifiers are noted, coaches are registered and any "new" coaches should now attend IMPACT clinic prior to performing any duties with athletes.
2) Prospective players complete forms on-line & submit to coaches with individual fees paid to the local club. Club director has check issued to Delta Volleyball for $35 individual fees plus $30 per team. Copies are made of medical history/release form & given to site director for practices in addition to team coach & club director copies. Players should NOT participate in ANY activities until registrations are completed & mailed to Region office. CLUB DIRECTORS: PLEASE REVIEW THE MEMBERSHIP FORMS FOR CLARITY...IF PARENTS FILLED OUT THE FORMS BY HAND, ENSURE THEY ARE LEGIBLE!!! Please encourage parents to utilize the on-line registration form...this may come in handy later if you have to utilize the medical release form.
3) Club Director selects other adults in program (who will attend large events where certified chaperones are required) who will serve as chaperones & submits completed forms & fees payable to Region. If these individuals are known at the beginning of the season, this should be completed early. National events require chaperones to present photo ID to pick up passes & allow team to compete.
4) Sanction of practice facility sent to Region office (see forms.)
5) Finally, locate volleyballs, check facility for safety, advise parents of security & safety issues regarding practices (in writing) and, at last, start coaching volleyball.
