Can't handle on-line registration? Computer acting up on you? Just don't want to put all that information on-line? You have an option.
 Here are the manual registration forms, which will cost you an additional $5.00 for manual processing. Forms must be completed legibly and fully. E-mail address is required to complete registration. (see link for free Hotmail account below.)
NOTE: The social security number is NOT Required for Junior Players (it IS required for coaches and chaperones in order to conduct background screening.) However, you may want to provide your daughter's social security number on your medical insurance form for proper identification in the event you are not present when medical care is rendered.
Junior Volleyball Players:
 2007-08 USAV Membership Form and Waiver of Liability. $40.00 fee for manual processing
Player Medical History and Release Form (junior players only) (this form will be held by the Club Director and a copy carried by team coach to all practices & competitions) Do not send this form to the Region office.
Be prepared to print both of these forms PLUS your membership renewal confirmation and take to your club director or coach. Do Not mail this information or your check directly to the Delta Region office.
Adult coaches and volunteers of J.O. Volleyball teams--Complete & sign the following forms:
for coaches, club directors & chaperones. New form each season.
Chaperone Form only if serving as a registered chaperone at events.
Club Contact Form Complete once each season for each club IF any background screenings will be ordered.
